Personal information about you or your child, such as address, date of birth, etc., is kept on a password protected computer database or in locked cabinets within the lab, which is also locked or monitored at all times. Only members of our lab have access to this information. This information will be kept for three years after the study is completed, or until you request that we destroy this information. The data you enter in this form will be sent to us over a secure, encrypted connection.

Research Results
Should you choose to participate in one of our studies, the data concerning your child that we collect during the study will be recorded on computer or paper with a unique identification number; your child’s name will not be stored with this information. We keep a table that matches the identification number with your child’s name, in case we need to contact you for further information at a later time. However, we will destroy this table within one year after the completion of the study. This table will remain in the locked lab at all times, until it is destroyed.
When the results of the research are published or discussed in conferences, no information will be included that would reveal your identity or your child’s.
