In order to receive your Dornsife Scholars Award\u00a0 funds, we will need the following:\u00a0 <\/strong><\/p>\n

Submit the information and documents as they become available. This will help to expedite the process.<\/p>\n

Send documents as attachments<\/strong>, not embedded in your email, to the Dornsife Scholars Coordinator (rmassari@usc.edu<\/a>).<\/p>\n

  1. A copy of your Dornsife Scholars Award offer letter from the Dean<\/li>\n
  2. Graduate School Acceptance Letter<\/li>\n
  3. Proof of Graduation from USC (your STARS Report or Transcript).\u00a0 If you do not have either document, alumni may order transcripts online<\/strong><\/a>.<\/li>\n
  4. Proof of Registration\/Enrollment in Courses (that shows your Course List and your Student ID# or Name)<\/li>\n
  5. Copy of Tuition Bill. Must include Student ID# or other indentifying information. (The bill should be detailed to show the expenses for which you are being charged, such as tuition, housing, etc.)<\/li>\n
  6. The exact start date of the semester you are entering.<\/li>\n
  7. Your personal email address so that we can reach you in the future. We acknowledge Dana and David Dornsife by providing updates on the accomplishments, successes and plans of our Dornsife Scholars.<\/li>\n<\/ol>\n

    For institutions other than USC, also include:<\/strong><\/p>\n

    It is the student’s responsibility to send the correct information–check with your university\/institution to confirm the information on their website. Contact their financial aid office or other office that would provide information about accepting funds for an award payment from USC to your student account.<\/p>\n

    1. The Remit to Name that should appear on the check. (Some institutions use, for example, “President and Trustees of ___ University.”\u00a0 Others use “___ University,”\u00a0 or\u00a0 “___ University, College of ___”,\u00a0 etc.)\u00a0 Be sure to get the correct name.<\/li>\n
    2. The Remit To Address\/Mailing Address. (Please note that the address on your tuition bill is not necessarily the correct address to send the check for this type of payment.\u00a0 To avoid payment delay, please contact the appropriate office for the correct mailing address to receive\u00a0 a check from our university for your student account).<\/li>\n
    3. The Official name and address of the institution. (This is sometimes different from the remit name and address.)<\/li>\n
    4. You must provide contact information (name, email, phone number) for the responsible person\/office at the receiving institution. This is necessary should we need to reach out to them for payment processing.\u00a0 Let them know that IF the institution needs to be set up as a USC “supplier” in our payment system, the contact person\/office will receive an email from USC with an on-line form for them to submit. To avoid delays, check with the institution for the correct contact person\/office\/email\/phone number.<\/li>\n<\/ol>\n
